JOB SUMMARY:

Penn State Health Outpatient Staff Pharmacists are responsible for assuring safe, rational, and cost effective drug therapy for patients within the Penn State Milton S. Hershey Medical Center by assuring the accuracy of drug dispensing, physician order evaluation, and the monitoring of drug therapy. Pharmacists are also responsible for communicating with all providers involved in the care of patients with respect to pharmacotherapy issues, participating in assuring adherence to Medical Center drug use policy, and providing pharmacotherapy education and information for physicians, nurses, residents, students, and patients.

The Pharmacy at the University Physician Center (UPC) fills prescriptions for patients discharged from our Medical Group practices, as well as acute (short-term) prescriptions for hospital employees. UPC Pharmacists take part in the Prescription Assistance Program to give patients medication counseling, assistance with medication costs and other prescription needs.

The Pharmacy Department Vision is to provide optimal pharmacotherapy to patients across the continuum of care through collaboration with and the education of our patients and healthcare providers. This will be achieved by implementing a patient focused pharmacy practice model, utilizing innovative technological solutions, conducting ground-breaking research, and fostering a spirit of excellence. Penn State Health utilizes an integrated, electronic medical record and an interdisciplinary approach to care.

MINIMUM QUALIFICATIONS:

B.S. or Pharm.D. degree in pharmacy required.

Current Pennsylvania Licensure or eligibility required.

Must be an effective communicator and have excellent time management and conflict resolution

PREFERRED QUALIFICATIONS

Preferred Specialty Pharmacy recent experience and/or NASP certification.
