Looking for an inspiring and exciting job opportunity? Join Penn State Health as a Clinical Case Manager for Cardiovascular Triage & Testing. The Clinical Case Manager for Cardiovascular Triage & Testing will offer you the opportunity to coordinate interdisciplinary professional care of the cardiac patient population as determined by individualized plan of care, clinical pathways, managed care guidelines and contracts. This role will offer various responsibilities including non-invasive cardiac testing, proficiency in EKG and IV skills, phone triage and emessaging, forms completion and pre-authorizations, assist providers when needed. The position will be based out of the Nyes Road Cardiovascular clinic.
Bachelor's degree in Nursing or Master's degree in Nursing required.
3 years clinical experience in specialty area required.
Currently licensed to practice as a registered nurse by Pennsylvania Board of Nurse Examiners required.
KNOWLEDGE, SKILLS, & ABILITIES:
Strong communications skills both oral and written required.
Strong organizational skills required.
Ability to analyze data reports on patient outcomes required.
Experience as a case manager, in discharge planning or insurance issues preferred.
Appropriate certification for specialty area preferred.
Proficiency in Microsoft Office preferred.
Motivational interviewing skills preferred.
